Pharmacia Patients In
Need (PIN) Program
Sometimes, affording
prescription medicines can be difficult. That’s why at Merck, we have created
the Patient Assistance Program—to keep affordable medicines within your reach.
This private and confidential program provides medicine free of charge for up
to one year to eligible individuals who do not have prescription drug coverage
and who, without our assistance, could not afford needed Merck medicines. Some
medicines covered include:
Clinoril, Decadron, Dolobid,
Fosamax, Indocin, Pepcid, Vioxx, Blocadren, Cosmegen, Cosopt, Cozaar,
Cuprimine, Desmer, Diuril, Elspar, Hyzaar, Lacrisert, Maxalt, Maxalt-MLT,
Mephyton, Mevacor, Midamore, Mintezol, Moduretic, Mustargen, Noroxin, Prinivil,
Prinzide, Propecia, Proscar, Singulair, Stromectol, Syprine, Timolide

The Pharmacy Savers Program
is a prescription drug discount program for individual Health Net Seniority
Plus and Health Net Life Medicare Supplement Plan members. This program applies
to members who do not have a pharmacy benefit, have a pharmacy benefit and have
exceeded their annual maximum, or members who have a generic-only prescription
benefit and want to access brand name products, not otherwise specifically
excluded or limited.(Members enrolled in
Health Net through an employer group, union or trust plan are not eligible for
this program.)
With Health Net Pharmacy
Savers, you receive discounts on your prescriptions just by using your Health
Net member identification card at participating pharmacies.

Pharmacia Patients In Need (PIN) Program
The Patients In Need Program
provide immediate assistance to patients who have limited income and are
uninsured or underinsured (e.g. no
prescription benefits).
Enrollment begins with the
physician, patient, or patient advocate calling the program at 1-800-242-7014,
between 8 am and 8 pm EST, Monday through Friday. Have the application with the
attached certificate in hand. Program specialists will collect specific
information about the patient, the drug therapy prescribed, and will ask for
the certificate number printed on the upper right hand corner of the
application. This application can be obtained by calling 1-800-242-7014,
going online at http://www.patientsinneed.com/general_info.asp
or by calling the Jackson Arthritis Center at 1-510-238-4851 (a limited
number is available).
If your application is
approved, a 30-day supply of the prescribed drug will be authorized. Although
there is no charge for the medication, there is a $5 dispensing fee to the
pharmacy for each prescription filled.
